Medical Cannabis

AdvaCare Clinics is a certified facility for the Minnesota Medical Cannabis Program. We perform evaluations, determine eligibility, register the patient into the Minnesota Board of Medical Cannabis Registration System, and provide a signed Medical Cannabis Certificate. The patient is then directed to the nearest Medical Cannabis Dispensary.

The Minnesota Medical Cannabis Certification is a service that is not covered under traditional medical insurance and therefore is a cash only option.

This program is limited to patients who are already a resident of the state of Minnesota with current photo documentation verifying Minnesota residency and who qualify with the following conditions:

  • Amyotrophic Lateral Sclerosis
  • Cancer/Cachexia (wasting and/or severe nausea)
  • Crohn’s Disease
  • Glaucoma
  • HIV/AIDS
  • Intractable Pain
  • PTSD (please understand, a diagnosis of depression or anxiety does not qualify as PTSD)
  • Seizures
  • Severe and persistent muscle spasms
  • Terminal illness with severe pain, nausea, and/or wasting
  • Tourette’s Syndrome

Patients can enroll starting July 1, 2018 and pick up medication August 1, 2018 for Autism and Obstructive Sleep Apnea.

We have developed a smooth process. You do you part, then we can do ours.

The Minnesota Medical Cannabis Certification is a service that is not covered under traditional medical insurance and therefore is a cash only option. There is NO insurance that will cover this. Our current fee for this evaluation is $250.00, payable in advance or on arrival with cash or credit card. We will not schedule your appointment until all of your requested records from other providers as well as our forms are completed and in our hands, ready for review. This can potentially save you a wasted trip to our clinic.

1. Download or request forms to be emailed (scheduling@advacaremn.com)

  • AdvaCare Patient Intake Form – fill out and send to AdvaCare Clinics
  • AdvaCare Records Release Form – fill out and send a copy back to Advacare so we can send on to any practitioner who is currently treating you for any of the above conditions. Please note: we must have received their records prior to your appointment. These documents must be reviewed and verified before we can schedule an appointment.
  • Minnesota Medical Cannabis Program Patient Email Acknowledge Form – This form must be sent to your practitioner at Advacare.

2. Call 952-835-6653 (AdvaCare Clinics) to schedule an appointment and bring clinic fee to the appointment.

There is a non-refundable fee of $250.00 payable to AdvaCare Clinics due at the first appointment. Please note: You may reschedule with prior notice, once this appointment is set, however if you do not show up for the first appointment, you must prepay to reschedule.

3. Access

You must have access to a camera or a camera-equipped mobile phone to capture/scan images of your IDs on the registry website.

4. Wait for Certification from Health Care Provider (Advacare Clinics)

  • When your practitioner has certified that you have a qualifying condition, you will receive an email (using the address you gave your provider) from the Office of Medical Cannabis.
  • The email will contain the link you need in order to register online and an important information sheet you should review before completing registration.

5. Register Online - In order to register, you will need:

  • Link in the email from the Office of Medical Cannabis. Your name, address and phone number.
  • Government-issued ID: state ID, driver's license, or passport (scan or photo from phone).
  • Government assistance plan ID: Medicare ID, Medicaid ID (scan or photo from phone).
  • Credit card to pay Annual Registration Fee via US Bank website. You will receive an email when your account has been approved.
  • You will need to complete the self-reporting form in your account in the registry. Complete the self-reporting form to expedite your visit to the cannabis patient center.

6. Visit Cannabis Patient Center

You, your parent, legal guardian or caregiver must visit the cannabis patient center, where a pharmacist will review your account, and recommend specific dosage and type. Payment will be made at that time, to the cannabis patient center.

7. Fill out Patient Self-evaluation

Before returning for more medical cannabis you must login to the registry and fill out the patient self-evaluation form. This must be done each time you need more medical cannabis.
