Medical Cannabis

AdvaCare Clinics is a certified facility for the Minnesota Medical Cannabis Program. We perform evaluations, determine eligibility, register the patient into the Minnesota Board of Medical Cannabis Registration System, and provide a signed Medical Cannabis Certificate. The patient is then directed to the nearest Medical Cannabis Dispensary.

The Minnesota Medical Cannabis Certification is a service that is not covered under traditional medical insurance and therefore is a cash only option.

This program is limited to patients who are already a resident of the state of Minnesota with current photo documentation verifying Minnesota residency and who qualify with the following conditions:

  • Amyotrophic Lateral Sclerosis
  • Cancer/Cachexia (wasting and/or severe nausea)
  • Crohn’s Disease
  • Glaucoma
  • HIV/AIDS
  • Intractable Pain
  • PTSD (please understand, a diagnosis of depression or anxiety does not qualify as PTSD)
  • Seizures
  • Severe and persistent muscle spasms
  • Terminal illness with severe pain, nausea, and/or wasting
  • Tourette’s Syndrome

Patients can enroll starting July 1, 2018 and pick up medication August 1, 2018 for Autism and Obstructive Sleep Apnea.

We have developed a smooth process. You do you part, then we can do ours.

The Minnesota Medical Cannabis Certification is a service that is not covered under traditional medical insurance and therefore is a cash only option. There is NO insurance that will cover this. Our current fee for this evaluation is $250.00, payable in advance or on arrival with cash or credit card. We will not schedule your appointment until all of your requested records from other providers as well as our forms are completed and in our hands, ready for review. This can potentially save you a wasted trip to our clinic.

1. Download the neccessary forms

Here are the steps to get certified by one or our registered Advacare providers:

  1. Read our Medical Cannabis information page to fully understand the two part process.
  2. Call 1-866-549-3756 or email scheduling@advacaremn.com to request a FREE telephone consult. Please include your name, telephone number and summary of conditions.
  3. We will call you to discuss your case.
  4. After our initial call, please download and complete our New Patient Intake Form, the Patient Info Release Form, and the Patient Evaluation Questionnaire. After completing these forms, please send them to us ASAP. It is fastest if you obtain your medical records that we deem necessary for your file review. We can also send for them, but it takes longer.
  5. Once we receive both your completed intake forms AND your required health records, we will review your file and deem it “clean” to proceed if appropriate. You are then considered pre-qualified, meaning you are now able to schedule a final face-to-face meeting with the health provider that can give you your certification for therapy.
  6. Download and complete this form from the State of Minnesota. It must be presented at the clinic visit. Download

At this time, you can schedule an appointment. We take checks (once cleared), cash or credit cards to hold your visit date and time.

2. Call 952-835-6653 (AdvaCare Clinics) to schedule an appointment and bring clinic fee to the appointment

There is a non-refundable fee of $250.00 payable to AdvaCare Clinics due at your scheduled appointment. Please note: if you schedule and pre-pay but cannot make the apppointment, we will allow you to reschedule with no penalty. However, if you are not pre-paid and cannot make the visit, the fee for the rescheduled visit will be $300.00

3. Access

You must have access to a camera or a camera-equipped mobile phone to capture/scan images of your IDs on the registry website.

4. Wait for Certification from Health Care Provider (Advacare Clinics)

  • When your practitioner has certified that you have a qualifying condition, you will receive an email (using the address you gave your provider) from the Office of Medical Cannabis.
  • The email will contain the link you need in order to register online and an important information sheet you should review before completing registration.

5. Register Online - In order to register, you will need:

  • Link in the email from the Office of Medical Cannabis. Your name, address and phone number.
  • Government-issued ID: state ID, driver's license, or passport (scan or photo from phone).
  • Government assistance plan ID: Medicare ID, Medicaid ID (scan or photo from phone).
  • Credit card to pay Annual Registration Fee via US Bank website. You will receive an email when your account has been approved.
  • You will need to complete the self-reporting form in your account in the registry. Complete the self-reporting form to expedite your visit to the cannabis patient center.

6. Visit Cannabis Patient Center

You, your parent, legal guardian or caregiver must visit the cannabis patient center, where a pharmacist will review your account, and recommend specific dosage and type. Payment will be made at that time, to the cannabis patient center.

7. Fill out Patient Self-evaluation

Before returning for more medical cannabis you must login to the registry and fill out the patient self-evaluation form. This must be done each time you need more medical cannabis.

AdvaCare Satellite Medical Cannabis Certification

JULY 2018 opens the doors to the AdvaCare “Satellite” Medical Cannabis Certification Clinics in Northern and Southern Minnesota.

We believe ALL OF YOU should have access to effective treatment options! While there are approximately 1200 doctors in Minnesota authorized to certify a patient’s qualifying condition so they can join the Medical Cannabis Registry, the overwhelming majority of them remain silent or are “invisible” because clinics fear they could lose managed care payments from federal streams of payment and pressure from prescription programs, as well as the fact it is hard to get clinics and doctors to adopt to a new way of approaching or thinking. “Fear of breaking the status quo”. But NOT the dedicated providers at Advacare Clinics. We support freedom of choice!

At this time, you can schedule an appointment by paying the $250.00 visit fee. We take checks (once cleared), cash or credit cards to hold your visit date and time. We will allow you the option to make a $150.00 down payment and then pay the balance (in cash), as we will not have the ability for CC or check payment at the location at your visit; however this increases the cost of the visit to $300.

The additional $150.00 would be due at the time of the appointment and you will not be seen if you come to the appointment without it. We really do not want to be handling money at these appointments; we’d rather focus on each scheduled patient. If you fail to come to the appointment, there are no refunds or rescheduling without another prepayment of the $150. In other words, if you paid the initial $250 and were to miss your appointment, you will be required to pay an additional $150 to reschedule.

Clinic Day Details

  • AdvaCare Satellite Clinics will usually be held on Tuesdays.
  • We will be bundling our appointments in groups of 25 on any given date. (We will not schedule a date to come to your area until we have a minimum of 25 paid and scheduled patients)
  • The northern clinic will be in the Moorhead / White Earth area, while the southern clinic will be in Rochester. The exact locations are still to be determined.
  • Our medical team currently consists of 2 professionals - they will not be screening nor qualifying anyone, just seeing the scheduled patients.

MEDICATION IS NOT DISPENSED AT THIS TIME, THIS IS ONLY PART 1 OF THE 2 PART PROCESS.
